Why do we do heart screenings? Our seemingly perfectly healthy son Zac died from an undetected heart condition when he was 17, one week shy of his 18th birthday.

Heart Screenings are for anyone between the ages of 10-25.

HEART SCREENINGS

Click Here to Sign up for a screening. Question O will take you to an external link to complete the health information, once you have completed the health information you must input the UUID in the space for question O and then click continue to confirm the appointment.
If you do not click continue the system will not confirm the appointment time. All confirmed appointments will receive a confirmation email with the appointment time.

Please review the information carefully on the scheduling page before reserving your date. ZMF heart screenings are typically scheduled for four hours, and thoughtful preparation ensures a smooth and successful event.

You must be able to secure volunteers if you are located outside our home base of St. Joseph, Elkhart, LaPorte, Porter, Marshall, or Starke County.

Thursday screenings are limited to the counties listed above.

A $50 deposit is required to secure a date.
